How to Register with us

We are an NHS family practice, providing general medical services to people living in Battersea and Clapham. You are welcome to register with us if you live in SW11, SW8 or SW4 within a mile of the surgery. (see map)

You can come into the surgery and fill in the registration forms. You will need to provide proof of address and your NHS number to register.

We also ask new patients to fill in a health questionnaire and then we offer a new patient check-up with a health care assistant or nurse. This gives us some basic information that will help in your future care.

You will be given two electronic forms to fill in. The GMS1 is the official NHS form needed for all GP registrations. The second form is our New Patient Questionnaire. This provides us with information about your health and lifestyle. We aim to put your details onto our system within 2 working days. We then invite you to make an appointment for a new patient check-up to complete the process. Please bring a urine specimen with you for this check-up, or be ready to produce one when you arrive.

If you have been in the UK for less than 6 months or have never been registered with an NHS GP, please ring the surgery before filling in these forms.
