If you would like to request a medical report, please contact the practice using our online form. If you are unable to use our online form, please call the practice after 10.30am and our staff will be able to help.

Our practice has decided to outsource the majority of our medical reporting work to an NHS Digital accredited company called MediData. MediData will be processing your medical report via eMR and providing online access via their secure encrypted portal. Other reports and letters are provided by the patients named GP. We provide reports as quickly as we can. Occasionally there may be a delay due to staff absence or for other reasons. We apologise for any inconvenience this may cause.

Medical reports and examinations are provided by the practice as an additional service that the NHS does not pay for. This means there will be a charge, to be paid before completion. Our Non-NHS work page provides more information about this.
